eXtreme Cheer US Finals Survival Kit Order Form
eXtreme will again be offering "Survival Kits" for US Finals. The basic Kit is $12 (which includes a personalized bucket and selection of snacks). To the basic Kit you may add a variety of items including a blanket, beach towel, photo album and cup. At this time the individual add-on items are not available by themselves - the basic Kit must also be purchased as this is what they will be packaged in. (Please note that some of the blanket colors may be limited.)
Please complete the order form below. Kits will be delivered during practice on Thursday, May 5. Payment may be made at that time via cash, check or credit card. More info to come on which practice I will be at during the beginning of practice and which practice I will be at during the end of practice.

NOTE: The photo album is a 5x7 album specifically created for either Thunder or Lightning with one page for each girl and several pages of candids. The Cup is a blue plastic cup with screw-on lid and straw.
